Fill out the form with your legal name, cell number, email, state, and the team member who referred you.

02Sign the Pilot Agreement
We email the Participation Agreement to the address you gave us. Review it and e-sign.

Once the signed agreement is back, we email a code tied to you and instructions for using it.

Create your account at timberhealth.co with the same name, email and cell from the form. Apply the code at checkout.

Applies to the first quarterly charge on a treatment program, the first three monthly program fees on Weight Management, and every lab panel ordered during the Pilot.
An independent licensed clinician in the Arora Health network reviews your intake and decides whether treatment is appropriate. Coaching is included with every program.

Participation is by invitation and governed by the Pilot Participation Agreement. Prescription products require an evaluation by a licensed clinician and are issued only if appropriate. Not guaranteed. If no prescription is issued, you automatically receive a full refund. Licensed providers make all medical decisions. Timber Health coordinates non-clinical services.
Not every program or lab panel is offered in every state. Weight Management medication is itemized separately, charged at the pharmacy's cost without markup, and not discounted. Programs are subscriptions that renew at the standard price after Pilot-priced charges; cancel any time from your account or with Customer Support.
